Recall Information

 

We are conducting a recall of certain UberNumb products that included a non-child-resistant cap. While no injuries or incidents have been reported, this recall is being conducted to assure our customers safety and to prevent any potential injuries due to the non-child-resistant cap. If you purchased one of the affected products, please immediately place the recalled topical anesthetic out of reach of children and follow the instructions below.

Remedy:

All affected products were all removed from distribution on or before April 26, 2018. For the affected cream products, we are replacing the current cap with a child-resistant cap at no charge. For the affected spray product, we are offering a replacement cream at no charge. Alternatively, for those who purchased a spray and do not want a replacement cream, we are offering a refund of the purchase price upon return of the unused portion of the spray.

 

Participation:

Call our toll-free number (302) 566-8237 for detailed information on how to obtain a replacement child-resistant cap for your recalled cream product, or a replacement cream or refund for your recalled spray. You can also send us a message at recall@uberscientific.com with the required information below and we will send you a replacement child-resistant cap. Even if your product appears to be safe, we are asking all customers to stop using the product immediately and participate in the recall.

Purchase Information:

To obtain a replacement child-resistant cap for an affected cream product by providing the name and shipping address used to purchase the affected product, please send us a message with:

• your name;

• contact number;

• current shipping address;

• the shipping address that was used to purchase the affected Uber Numb cream; and

• the UPC and lot number from the affected Uber Numb cream.

Provided the name and shipping address used to purchase the affected product match our records we will send your replacement cap to the current shipping address you provide. If the information provided does not match our records, we will contact you regarding other options.

 

Proof of Purchase:

To obtain a replacement child-resistant cap for an affected cream product by providing proof of purchase, please send us a message with:

• your name;

• contact number;

• current shipping address;

• proof of purchase; and

• the UPC and lot number from the affected Uber Numb cream.

Upon confirming your proof of purchase we will send your replacement child-resistant cap to the current shipping address you provide.

 

Returning Product:

To obtain a replacement child-resistant cap for an affected cream product without providing the original shipping address or proof of purchase, please send us a message with:

• your name;

• contact number;

• current shipping address;

• the type of affected product you purchased; and

• the UPC and lot number from the affected Uber Numb cream.

Upon receiving your information we will send a prepaid shipping label to the current shipping address you provide. Using the prepaid shipping label, please send the affected product to us and we will return the product with replacement child-resistant packaging.

 

Information required (Spray):

To obtain a replacement cream or refund for an affected spray product, please send us a message with:

• your name;

• contact number;

• current shipping address;

• the UPC and lot number from the affected Uber Numb spray; and

• desired remedy (replacement cream or refund).

Upon receiving your information we will send a prepaid shipping label to the current shipping address you provide. Using the prepaid shipping label, please send the affected product to us and we will return a replacement cream or refund once the affected spray has been received. Please note that refunds are being offered for spray products only. For cream products the current lid is being replaced with a child-resistant lid.
